To Apply for Membership with Rainbow Medicinal, there are two ways:

1. If you are currently a holder of a ATP, Federal license from Health Canada (MMAR), you can apply in person at our office.

2. If you don’t currently have an ATP (MMAR) license (or don’t know what that is) then you can apply with the form listed below. Please download and print the Rainbow PDF Application listed below. Then take this form to your doctor so they can fill it in. Once the form is completed, please return it to Rainbow for registration.

This is the form your doctor will sign, releasing your medical information to us, and to complete the application with the official stamp and contact numbers for their office. Then bring this form to Rainbow office so we can start the registration process.

PLEASE NOTE: We encourage all our members to obtain a Federal Card from Health Canada (under the MMAR Regulations). We are providing you with access to forms, you can pick the up in person from our office, or download them from the link provided above. Our staff will offer you guidance with your application and any assistance you may need through the application process.
