Application and Health Canada Forms

PLEASE NOTE: We assist all our members to obtain a Federal Card from Health Canada (under the MMAR regulations). We are able to provide you these documents from Health Canada. You can pick up the documents in person from our office, or download them from our website. Our knowledgeable staff will offer you guidance and assist you through the process.

If you do not have access to a printer please call us at 416.927.8639 and make an appointment to come and pick up these documents in person.

There are two ways to obtain membership: 1.   If you are currently a holder of a ATP card, Federal license from Health Canada (MMAR), you can call us at 416.927.8639 and arrange to apply in person at our office located at Church and Wellesley.       2a.   If you don’t currently have an ATP (MMAR) license (or don’t know what that is) then you can apply by going to Health Canada’s site.       2b.   Download and print the Rainbow PDF Application       2c.   Click here for “Release From Liability” form for your doctor to sign provided by Canadian Medical Protective Association       2e.   Once the form is completed, please return to Rainbow by fax at 416.927.1164 or call us at 416.927.8639 to arrange an intake appointment.
